TRS-ActiveCare Transition Complete
Effective September 1, 2026, Conroe ISD officially transitioned from our self-funded health plan to TRS-ActiveCare. All medical and pharmacy benefits are now administered through the statewide plan, providing our employees and their families access to reliable, comprehensive, and high-quality healthcare coverage.
Next Steps for Your New Benefits
Present New ID Cards: Show your new Blue Cross and Blue Shield of Texas (BCBSTX) card for all medical care and your Evernorth/Express Scripts card for all pharmacy pick-ups.
Verify Your Primary Care Physician (PCP): Check the PCP listed on your BCBSTX ID card if you are enrolled in TRS-ActiveCare Primary or TRS-ActiveCare Primary+. If a change is needed, call the Customer Service number on the back of your card.
Referrals and Ongoing Care: If you are enrolled in Primary or Primary+, most specialist visits require a referral from your PCP (Note: HD members do not need referrals). If you have upcoming treatments or procedures, confirm with your provider that any required referrals or authorizations have been submitted under your new TRS-ActiveCare plan.
Mail Order Prescriptions: If you have refills remaining on a prescription, call Express Scripts at 1-844-367-6108 (available 24/7) for assistance with transferring your prescription.
Specialty Medications: Call Accredo Specialty Pharmacy at 1-800-596-7701 to transfer specialty prescriptions..

Benefits Updates
Important Health Coverage Tax Documents (FORM 1095-C)
Conroe ISD uses the IRS's alternative method for furnishing Form 1095-C. Because of this, we do not automatically send Form 1095-C to all employees or plan participants.
If, during any month of the calendar year, you were employed and worked at least 130 hours, or if you were enrolled in our self-funded health plan, you may request your Form 1095-C at any time.
To request your Form 1095-C, contact the Benefits Office at benefitsoffice@conroeisd.net or 936-709-7808. Requested forms will be provided within 30 days of your request or by January 31 of the following year, whichever is later.
For your convenience, an electronic copy is also available in the Employee Access Center for current employees. This electronic version does not replace your right to request a printed copy from us.
